The Membership Agreement has an initial term of one year and continues for successive one-year terms unless either party gives thirty (30) days' written notice of non-renewal before the end of the current term.
Either party may terminate the Membership Agreement on thirty (30) days' prior written notice. To cancel, email [email protected] or call (702) 580-7605.
On termination, the remainder of the Periodic Fee is refunded on a prorated basis, prorated daily as of the effective date of termination, less the value of any services received before that date.
The Membership Agreement terminates automatically if the Periodic Fee is fifteen (15) days overdue.
We ask that you reschedule or cancel home visits and appointments at least 24 hours in advance so we can offer the time to other patients.
For safety and regulatory reasons, prescribed items that have been dispensed or shipped cannot be returned or refunded, except where required by law or in the event of a dispensing error on our part.
If you believe you have been billed in error, contact us within 30 days of the charge and we will review and correct any confirmed error promptly.
Your signed Membership Agreement sets out the full cancellation and refund terms and governs if anything on this page differs from it.
